BILLING POLICIES

1) If you have changed your address, telephone number or employment, please notify our business office of your new information.

2) We participate with Medicare.

3) We verify insurance coverage and benefits.

4) We will file your charges with your current insurance company. We will collect payment for any deductibles, co-pays and any out-of network fees at the time of your visit.

5) If you are enrolled in an HMO plan, and you change your Primary Care Physician (PCP) at any point, please contact our office with the new physician’s name. We will make every attempt to obtain referrals/authorizations from your primary care physician, but it is ultimately the patient’s responsibility. Please contact our Business Office prior to your visit to ensure a referral/authorization is in place.

6) We will pre-certify scheduled hospital admissions. If you are admitted to a hospital after hours to Dr. Martinez’ service, please contact your insurance company immediately. The phone number is listed on the back of your insurance card. Once you notify your company, they will contact our office or the hospital for clinical information regarding your admission.

7) If your insurance terminates, please contact our business office immediately. If you have new coverage, contact our business office with the new information.

8) Medicaid patients must present a current card at the first visit of every month. Your appointment may be rescheduled if this information is not available at the time of your visit

9) Self-pay accounts (No Insurance) must remit payment in full at the time of each visit.

10) Please do not hesitate to contact our Business Office for assistance with your account and insurance needs.
