OFFICE POLICIES

1) It is very important you bring ALL medications to each visit, regardless of prescribing physician. If you are taking herbal supplements or vitamins, bring them with you also. Your appointment may be rescheduled if this information is not available at the time of your visit.

2 ) Always sign in at the check-in window upon arrival.

3) Make sure you always check out with the front office to ensure all additional appointments are made and further testing is scheduled if necessary.

4) Our phones are answered 24 hours. Local (806) 793-8310 or Toll-free (866) 793-8310. Instructions are enclosed regarding the use of our auto-attendant. Our staff will return your call as quickly as possible.

5) Only one person may be designated as the official spokesperson for the patient. A properly executed authorization to release information must be in our files. At the patient’s request, visits may be tape-recorded for the patient to share with other family members.

6) If the patient is non-ambulatory or extremely weak, a caregiver is required to be present at all times. The caregiver must be able to assist the patient in/out of vehicle/wheelchair/exam bed. Due to OSHA Safety regulations, our staff may not lift patients. Arrangements may be made with our office to see a non-ambulatory or bed bound patient in the emergency room as needed.

7) If the patient is in a nursing home, the patient must be accompanied by a family member, caregiver, or nursing home representative.

8) Sprite and crackers are available for chemotherapy patients experiencing nausea. Coffee is available for chemotherapy patients and their family member who may accompany them. Patients may wish to bring their own lap blanket for comfort.

9) NO children are allowed past the lobby doors. Do not leave children unattended in the lobby area.

10) As a courtesy to the physician and patients, all cellular phones and pagers must be turned off while in the exam room.

11) Prescription refill requests must be called in before noon Monday through Friday. Your pharmacist is welcomed to fax a refill request to (806) 793-7874. Since some insurance carriers require preauthorization for medication refills, which can take several days, it is a good idea to call in the prescription 3-4 days before the medication runs out.

12) Patients must always use the front doors; back entrances are for authorized personnel only.

13) Parking is available directly in front and in back of the Lubbock Cancer Center building only. The parking lot across the street to the south is private property - vehicles will be towed. When parking on the front curb, there must be a 3’ space from the end of your vehicle to the driveway entrance. The City of Lubbock will issue tickets for a violation.

14) Out-of-town patients must understand that we make every attempt to schedule diagnostic tests (CT, MRI, etc.) and the office visit on the same day for the patient’s convenience. Dr. Martinez requests your understanding and patience if you experience a lengthy waiting period for the office visit. We are awaiting results.

15)  All results of lab tests, scans, etc. will be addressed face-to-face with Dr. Martinez at the next follow-up visit, unless otherwise specified by Dr. Martinez.
